Abstract

Target. To study the relationship between the level of HDL C and 30-year survival in cohorts of men 40-59 years old with different status of cardiovascular diseases (CVD) using the survival function. Material and methods. Four representative samples of men aged 40-59 years were formed randomly from the inhabitants of the Metallurgichesky district of Chelyabinsk, differing in health status: practically healthy [ZD, 174 persons], patients with hypertension [G, 149 persons] who had suffered a heart attack myocardium [I, 198 people], patients with a combination of myocardial infarction and hypertension [IG, 154 people]. The cut-off points of the observation period were 0 and 30 years. The end point was the fact of death. in these cohorts, the functions of 30-year survival were assessed according to Kaplan-Meier. Confidence bands of the survival function were constructed on the basis of nonparametric Kolmogorov-Smirnov tests. Based on the survival function and the values ​​of cholesterol-HDL cholesterol, a distribution function was constructed using the Cox model. Results. The influence of the initial concentrations of HDL-C on the function of 30-year survival in cohorts of men 40-59 years (545 people) was shown, regardless of the presence of hypertension and / or myocardial infarction. An increase in the concentration of HDL-C from 0.3 to 1.7 mmol / L in cohorts is accompanied by a significant increase in survival. Conclusion. The characteristics of the functions of 30-year survival in the studied cohorts of men 40-59 years old with an increase in the level of HDL-C demonstrate heterogeneity in the onset, duration of periods of significant differences in survival curves, which indicates the effect of HDL-C concentrations on survival functions in the cohorts, the nature of the influence Depends on CVD status and follow-up periods.
